GSS to participate in Stratis Health’s Initiative to improve transitions of care

Good Samaritan Society (GSS) – Communities of Brainerd and Pine River along with GSS Home Care and Hospice continue to work to provide patients with the best and safest care possible and were recently selected to participate in a quality improvement initiative to improve care coordination across settings of care and to reduce avoidable hospital readmissions. The initiative is supported by the federal Centers for Medicare and Medicaid Services (CMS), and is led by Stratis Health, in its role as Minnesota’s Medicare Quality Improvement Organization.

As part of the initiative, Stratis Health is convening community-based organizations to come together as teams for learning, sharing and networking. These teams will have access to quality improvement experts and evidence-based practices. To address the issue of care transitions and avoidable hospital readmissions, they will identify community-specific causes for poor transitions, engage community stakeholders as partners and implement evidence-based interventions.

“We are committed to continually improving quality in our patient rehabilitation process to ensure safe and successful transitions back to daily living,” GSS local executive director Mike Deuth said. “This partnership with other local healthcare providers is just one more way we can come together as a healthcare community to work for the best outcomes for each patient.”

GSS staff participating in the collaboration include Brainerd’s Bethany director of nursing, Shelley Denny; Nisswa’s Home Care and Hospice director Kayla Farr; Pine River’s director of nursing Vickie Howe; and Brainerd’s Woodland director of nursing Marie Michlitsch.

The GSS is part of The Evangelical Lutheran Good Samaritan Society, a not-for-profit organization that owns and operates more than 240 long-term care, rehabilitation and retirement living centers across the country. The Good Samaritan Society’s mission is to share God’s love in word and deed by providing shelter and supportive services to older persons and others in need, believing that “In Christ’s Love, Everyone Is Someone.”
