Good Samaritan Society takes part in health’s initiative

Good Samaritan Society – communities of Brainerd and Pine River along with Good Samaritan Society Home Care and Hospice were recently selected to participate in a quality improvement initiative to improve care coordination across settings of care and to reduce avoidable hospital readmissions. The initiative is supported by the federal Centers for Medicare & Medicaid Services (CMS), and is led by Stratis Health, in its role as Minnesota’s Medicare Quality Improvement Organization.

As part of the initiative, Stratis Health is convening community-based organizations to come together as teams for learning, sharing and networking. These teams will have access to quality improvement experts and evidence-based practices. To address the issue of care transitions and avoidable hospital readmissions, they will identify community-specific causes for poor transitions, engage community stakeholders as partners and implement evidence-based interventions.

“We are committed to continually improving quality in our patient rehabilitation process to ensure safe and successful transitions back to daily living,” says Good Samaritan Society local executive director Mike Deuth. “This partnership with other local health care providers is just one more way we can come together as a health care community to work for the best outcomes for each patient.”

Good Samaritan Society Staff participating in the collaboration include: Brainerd – Bethany Director of Nursing Shelley Denny, Nisswa – Home Care and Hospice Director Kayla Farr, Pine River Director of Nursing Vickie Howe, and Brainerd – Woodland Director of Nursing Marie Michlitsch.
