In honor of National Healthcare Decision Day last week, Cuyuna Area Connections will offer two virtual learning opportunities to learn about Honoring Choices, the advance care plan document that should be filled out to prepare families for traumatic events.

The document is something that should be completed when families and friends can come together in a calm, safe space and plan for the end of life. The sessions will also cover how to get an advance care plan on file with a primary care provider.

The first virtual session, hosted by Cuyuna Regional Medical Center staff, is at 11 a.m. Wednesday, April 21. The link for registration is https://bit.ly/3sBto0L .

The second session, hosted by a Central Lakes College nursing instructor and students, is at noon Wednesday, April 28. To register, visit https://bit.ly/3tDLmku .

A recording of last month’s session on social work and care management services is available at https://bit.ly/2QCCrRU . Visit surveymonkey.com/r/HX5DRBQ to give feedback on the social work session.
